Health Services
STUDENT HEALTH SERVICES:
Health care: Providing first aid, emergency care, medication administration, and other direct nursing services.
Health teaching: Educating students and faculty on health, hygiene, and disease prevention.
Assessment and evaluation: Planning, assessing, intervening, evaluating, and reassessing students with health conditions or disabilities.
Collaboration: Working with students, families, school personnel, and community agencies.
IMMUNIZATION REQUIREMENTS:
Minnesota School Immunization Law requires all students enrolled in grades Kindergarten through 12th grade to show they have received required immunizations or have exemption paperwork on file. Public school districts are required by MN law ( MS 121A.15 ) to maintain immunization records for all students, including homeschooled students.
MEDICATION:
Parents may request that the school district administer medication to their child when it is necessary for the medication to be administered during the school day.
Prescription Medication:
A Medication Administration Form is required to be filled out each school year, signed by a physician/licensed medical provider and parent/guardian, and turned into the Health Office.
The medication needs to be brought in the original container that is labeled by the pharmacy.
Non-prescription Medication:
The Health Office does not have Tylenol/ Motrin or any other over-the-counter medication to distribute to students.

The medication needs to be supplied to the school and brought in the original container.
IS MY CHILD WELL ENOUGH TO BE AT SCHOOL?
Please keep your child home from school if he or she has any of the following:
Temperature of 100.0 degrees or higher: Students need to have a temperature below 100.0, without fever reducing medications for 24 hours before returning to school.
Vomiting or diarrhea: Keep students home for 24 hours after the last episode.
Rash (disease related) or of unknown cause: Check with your student’s healthcare provider before sending your child to school.
Communicable disease (such as strep. throat, chickenpox, shingles, impetigo, scarlet fever, pink eye, etc.): Please notify the health office with any of these diagnoses. If a throat culture has been done for strep, please keep your student home until the results are back or they have received antibiotics for 24 hours.
STUDENT HEALTH PLANS:
Student Health Plans are a guide to provide the student's health care needs while in the school setting, with instruction on addressing healthcare needs or appropriate response to a student’s healthcare issue.
Student Health Plan Forms (Asthma, Allergy/Anaphylaxis, Cardiac, Diabetes, and Seizures- a student health plan needs to be filled out each school year and signed by a physician/licensed medical provider and parent/guardian.
Update health conditions for your student in Synergy- ParentVue and notify the Health Office regarding the health condition(s).
SCREENINGS:
Vision and Hearing health screenings are offered at age appropriate levels during the school year, typically in the fall. These screenings follow guidelines developed by the MN Department of Health. If you would like your child to participate in a screening, please contact the Health Office.
DENTAL SERVICES:
Comprehensive dental care is offered through Family HealthCare’s Mobile Dental Unit.
